West Indies were unchanged from the team that also hit 13 sixes to win the first T20 by seven wickets at the same ground last Friday. South Africa replaced left-handed pacer Nandre Burger with Lizaad Williams, who took 3-36.
The T20 that closes the series will be on Tuesday, also in Tarouba.
South Africa prevailed in the previous two-match Test series against the West Indies, winning the second Test in Providence, Guyana, after a rain-tied first match in Port of Spain, Trinidad.
